I crushed a primer once because I was rushing and my hands were numb. Luckily nothing went wrong but it scared me. Ever since then, I reload super slow and focus more...

I have crushed primers several times, that's because the primer pocket was not deep enough. So I then started using a "primer pocket rimmer" and from then on there were no more crushed primers. Getting the pocket the right depth is important!
